New Patients
How do I process a new patient?
Summary
Warmly welcome the patient, complete required paperwork and identification, log them as a new patient, prepare their chart materials, take pre-exam vitals, and hand off to the doctor.
- 1Stand and greet the patient warmly by name as they arrive.
- 2Have the patient sign the Sign-In Sheet (parent or guardian signs for a minor).

- 3Mark the Appointment Book by coloring the box red to the left of the name.
- 4Copy the patient's photo ID; for minors, copy the parent's ID and any guardianship papers if the parent is not present.
- 5Assign a new patient number from the New Patient Log.

- 6Notify the marketer that the patient has arrived.
- 7Complete all required forms and have the patient sign and date them; witness only the forms the patient actually signed.

- 8Fill in the Travel Card and copy the Confidential Patient Information (CPI) form for chart completion.

- 9Organize the clipboard with Travel Card, CPI, informed consent, and Patient Affidavit.
- 10Escort the patient to the exam room and complete the pre-exam information: age, height, weight, blood pressure, pulse, and x-ray computer entry.
- 11Place the clipboard in the hot file and notify the doctor.
Do not skip x-rays or therapies solely because a late new patient arrived late.
