    In Medisoft, go to the Lists menu > Patients/Guarantors and Cases. The Patient List screen appears.

    With the Patient radio button selected at the top of the Patient List, click New at the bottom of the screen to open the Patient/Guarantor screen.

    On the Name, Address tab, enter the chart number (new-patient number), name, address, birth date and phone from the CPI form (use general terms here — never paste protected information into this assistant).

    Click the Other Information tab to set the assigned provider and referral source noted by the marketer, then click Save.

    Back on the Patient List, select the Case radio button and click New to create the case; on the Personal tab enter the case description, guarantor and marital/employment details, then use the Diagnosis tab to enter the diagnosis codes and click Save.

    Go to the Activities menu > Enter Transactions (or click the Transaction Entry speed button on the toolbar). The Transaction Entry screen appears.
