What do I document on the Travel Card for therapy, and when do I stop and get the doctor?

Summary
The Travel Card travels with the patient and starts the closing procedure. Services must match the Physical Medicine Orders before anything is posted, and any mismatch stops the process.
- 1Therapeutic exercise is a set of repetitive stretching or strengthening maneuvers to develop weakened or injured muscle groups that support the spine. Script: “Repetitive maneuvers to stretch or strengthen weakened muscles.”
- 2Therapeutic exercise is timed: minimum 8 minutes, maximum 15 minutes, and is not typically performed on the same visit as ultrasound and massage.
- 3The Travel Card travels with the patient throughout each office visit and is the starting point of the closing procedure.
- 4The completely documented Travel Card is presented to the front desk CA when the patient schedules the next appointment.
- 5Confirm all services provided and have the patient sign the Travel Card verifying receipt, or confirm the patient and Doctor have signed and dated it.
- 6The services provided must match the Physical Medicine Orders before you post anything.
- 7Immediately post services and charges to the Gold Card, Medisoft, and the Appointment Book, in that order — never hold a stack of Travel Cards to enter later.
- 8Never provide a therapy that is not prescribed on the Travel Card, and never change the manner (location, time) in which an ordered therapy is provided.
- 9If documented services do not match the Physical Medicine Orders, stop and resolve the discrepancy before posting or continuing.
- 10If a genuine operational or safety issue prevents completing an ordered therapy, pause and escalate to the Doctor or manager rather than changing or omitting the order.
