How do I request and log petty cash?

Summary
Only the Clinic Director handles petty cash. They count the cash, match it to the ledger, and email the request with the ledger and receipts to the Management Office.
- 1Only the Clinic Director (Doctor) may complete the Request For Petty Cash Form; clinic staff are not involved with petty cash.
- 2The Clinic Director counts all available petty cash and matches it against the balance on the petty cash ledger.
- 3The Clinic Director emails the Management Office the Petty Cash Request Form, a copy of the Petty Cash Ledger, and copies of all receipts in the order logged on the ledger.
- 4The amount requested should be the difference between the present balance and the clinic average.
- 5Note the petty cash request amount on the ledger on a separate line with the date of the request.
- 6When the petty cash check is cashed, note the date cashed and the amount in that same space.
- 7Each ledger line lists date, name/description, debit, deposit, and new balance so the running total stays accurate.
