How do I pink a patient when they are released from care?

On the release visit, make the confirmation call to the adjuster or attorney with the patient present, have the patient complete the Patient Satisfaction Questionnaire, then complete the Patient Discharge Information (Pink Slip) and log the patient before the chart moves to the Final Packet basket.

    While the patient is with the doctor for the final re-exam, pull up the case in Medisoft and record the attorney or adjuster contact information and claim number.

    Ask the patient to sign and date the daily treatment note and complete the Patient Satisfaction Questionnaire at checkout; file the completed questionnaire on the left side of the chart.

    If the patient gives a negative review, email the questionnaire to Quality Assurance immediately.

    With the patient present, call the adjuster or attorney on speaker phone to advise the patient is being released. Do not call the adjuster if the patient is attorney represented.

    Tell the carrier or attorney the bill and records will be sent within 2 days and ask if anything else is needed to begin the settlement/demand. If leaving a message, request a callback to the patient and give the patient the contact information.

    Complete the Patient Discharge Information (Pink Slip): patient name, today's date, current balance due, and reason for discharge (PRN, non-compliance, no coverage, referred elsewhere). Do not sign 'Logged By' yet.

    Enter final services on the Patient Ledger (Gold) Card, in Medisoft, and in the Appointment Book, then print a Medisoft ledger and place it in the chart.

    Staple the Pink Slip to the top right of the Patient Ledger (Gold) Card.

    Log the patient on the Final Packet Log with name, account number, lien date, and date pinked. If no Final Packet will be sent, put NA in the Final Packet boxes and note the reason.

    Only after logging, sign and date the 'Logged By CA' area of the Pink Slip, matching the pinked date.

    The doctor signs and dates 'Approved by D.C.' after the physician report is completed.

    Place the chart in the Final Packet basket/pocket and follow the state-specific Final Packet process.
Important

Every patient released from care must be pinked before the chart is filed as inactive. Sign 'Logged By CA' only after the patient is on the Final Packet Log.
