Summary
Correct paper records with a single line and a note — never correction tape or erasable ink. Minors are consented by a parent or legal guardian, and any indication of treatment with another provider goes to the doctor.

- 2After daily closing, Medisoft corrections require director involvement.
- 3Never conceal a discrepancy merely to make totals appear to match.
- 4A minor's consent must be completed by the mother, father, or legal guardian; retain guardianship papers when applicable.
- 5Official ID should be obtained by the third visit; if unavailable, follow the manual's approved alternative proof procedure.
- 6If the Patient Affidavit indicates treatment with another provider, send the affidavit and the Statement of Patient Change of Treating Doctor to the doctor for review — do not assume the patient may receive duplicate treatment.
- 7For questionable contingency cases, look for reasons to treat but do not independently approve or decline; complete paperwork, place the patient privately in an exam room, alert the doctor, and call a director away from other patients.
- 8Only after director instruction should the patient be informed that contingency treatment cannot be offered.
